Device unlock request approved but no email

I have Samsung Galaxy S4 and locked to ATT. I have submitted 2 requests for same device as non-ATT customer and after confirmation, both the requests are approved but I did not get the email with unlock code. Tried 4 customer service representatives, 1 tech support and 1 sales/billing but no one is aware of anything. How do I get my approved unlock code?

Today, I submitted 3rd request and finally got the approval email. Seems like ATT emails are "lost" on their side and you need to keep trying to get the email.
