Where do I send complaints!?

I've been dealing with a fraud issue with an AT&T employee.  They canceled my account and opened a new one in my name.  I've made several phone calls over the last few days and was assured everthing was taken care of but today the tech showed up to install my "new" service.  I've had U-verse for almost 2 years!  I've requested follow-up from management and the address to where I can send a formal complaint but have received nothing.  I don't feel that anyone I talk to is taking this issue seriously.  Please let me know where I can send a complaint.
