I found a similar issue with the Optical Digital Audio Output on the DVR (http://utalk.att.com/utalk/board/message?board.id=Uverse_TV_Equipment&message.id=22250#M22250), so maybe it affects the HDMI Digital Audio Output as well. Any loss or confusion of the "Handshake" will cause the DVR to shut down all Digital Audio Output. If the TV has a Digital Audio Input (either Optical or RCA), You might try running a cable for that as oppsed to a new (and much larger) HDMI cable to see if it is strictly an audio over HDMI issue. One thing you don't mention is, if the picture drops as well using the switch box. If you are not losing picture, maybe the switch box is not new enough (HDMI Revision) to handle the way that Uverse sends audio over the HDMI connection.
