Replacemen t phone billing issue...ov er 2 weeks and 8+ hrs on hold!!!!!! !!!!!

My wife has been on the phone with AT&T reps over 7 times in the past two weeks and has wasted 8hrs of her time waiting on hold for a supervisor tonight. In the past week she has had 2 supervisors PROMISE to call her back to fix our issue.
The story:
Bought a new Samsung Galaxy III within the 30day I called to have it replaced. The customer care rep said that our bill would be charged the full replacement price of the phone, until AT&T received my old phone and then it would be taken off. Well.....when they ATT&T received the phone they only credited us HALF the full price....BUT credited our credit card.....not our bill. So we have a $583 charge on our bill that is not being taken care of. There have been:
-2 supervisors that would say they would call her back......and NEVER did
-called 8 time in the past two weeks to talk to someone to help
-she has wasted over 8 HOURS waiting on hold and repeating herself over, and over
-she has been on the phone since 6:30 and it is now 8:45 (September 17th) on still nothing...has been "transferred" to a supervisor but NOTHING!!!
I need to know who I can call....OTHER then customer service to get the problem resolved, this is complete non-sense. I am tired of wasting our time over the past two weeks just to be put on hold.
Someone PLEASE give me a phone number of Corporate AT&T

AT&T's return policy has changed effective 10/07/12. You have 14 days to return the device & cxl without a ETF. Policy info HERE.
Where is Justin Timberlake when you need him?. The supervisor finally answered and now put her on hold....this is COMPLETELY RIDICULOUS...I have gotten a faster response from this forum then my wife being on the phone. THIS IS POOR customer service. I will relay my first message to the lady you spoke of....is it sad that it has to come to this....some from 2 weeks ago should have taken care of this and done their job.!!!!!!!

I am very sorry this has been such a pain for you. I got your PM and responded just now. Will you please take a look (little blue envelope, upper right hand corner) and reply when you get a moment? I need just a bit more info.
