I've owned my 920 since November and have had no issues with it at all. I updated to the latest version of the OS when it first became available and also experienced no bugs or anything of the sort. Up until today though, I launched my camera and the camera was distorted and glitchy. I took a screenshot for reference (below):
I powered the phone off and back on and when I tried the camera again I now had my camera options on the UI greyed out and a blank screen (below)
After trying the Volume Down+Camera button reset that is recommended for most phone errors and having it not resolve my issue, I decided to do a factory reset (Settings>About>Reset) hoping it will resolve the issue but it did not.
Also, the device itself is in pristine condition, not once taken any damage, and is kept in an Otterbox Defender. I tested the front facing camera by using a photoapp (Camwow) to verify that it still works and it does. I also tried to use different flashlight apps that utilize the LED flash and they all failed to work.
Any help or suggestions would be appreciated, thank you.

That's clearly a hardware failure. You'll need to contact AT&T or Nokia about getting it repaired/exchanged under warranty.

Because of a shortage of 920s in stock, you might be better off going through Nokia directly. Try your local AT&T corp store first of course, but if you're told that you'll have to wait more than 14 days for a replacement, hit up Nokia.
