How do I get AT&T to provide a letter stating that a non-paymen t issue has been resolved?

I am in shock. I just learned that AT&T sent me to collections for $225 bill that they acknowledge I never received. I was completely unaware of the outstanding amount. They have my phone number, and being a phone company, it seems they would have called me before sending me to collections.
I am in jeopardy of losing a home I"m trying to buy since my credit score dropped 100+ points over this one issue on an otherwise blemish-free credit history.
This account was contentious from nearly the start since it was for internet service that was never installed, or never worked. We couldn't get it resolved, so we disconnected and the account was closed in April. I thought all was paid in full and because we never recevied another bill, never thought about it again--until this week when I went to get loan approval for my home.
I am very upset and want AT&T to issue me a letter clearing this matter and indicating that it is paid in full, completely resolved and have it removed from my credit report.
I don't know who to talk to about this matter. Any direction on whom to contact?
