PUK Code--Wher e is it?

I locked myself out of my GoPhone and have 2 remaining attempts to unlock the phone with the PUK Code. I know it is 8 digits. I have the SIM card and SIM Card Certificate (credit card size). I fell into a crack in the system...
On the SIM Card Certificate, I have the SKU, UPC, and ICCID numbers. Then I have something above where the sim card was, that says "RS#: _ _ - _ _ _ _" (I am not going to give the numbers out). On the bottom right, I have "GOP MS T _ _ _ _ _ _ _ _ D" I tried those numbers, but they didn't work. I also have the IMEI Number, And the S/N number. I even have the owners manual for my phone. None of these things have the PUK Code.
I tried to log on online with my 10-digit number and 4-digit password, but it says "The system could not log you on. Please enter your password and try again." I tried to access it through email, but that did not work. I called 1-800-331-0500 (Customer Service), I went through the prompts, then they asked for my PIN Code, and I entered it (correctly) but they said it did not match my records. I know my pin, which is _ _ _ _.......How did I fall into this crack???
I cannot talk to a real person, for some reason...I pay a lot of money on my phone and now I am blocked from using it for accidentally changing my PIN?
So my final questions are:
• Where is the PUK Code located in my starter pack.
• What is a number I can call to actually speak to a human being.
• Why can't I log onto my online account.
• Why doesn't my PIN work when I call 1-800-331-0500.

But since I have a GoPhone, should I call a different number? Or not? And when I do call, how do I get to an actual person?

I just came home from Radio Shack with my activated LG Neon phone. Turned it off, turned it on, and it is asking me for a PUK code. Nothing works...not 0000, not 1234, nothing.
I want to puke because now I don't even have a phone!!!!! Please help. This is brand new - not even 30 minutes old and I'm locked out??????? Thanks.

I need my puk code, but I need my pin code to access anything. I don't even know my pin because it kept saying it was wrong when I used the last four digits of my number, even though that's what my instructions were. So now I have no idea what my pin code is and I don't know what in the world to do!
