GoPhone, cannot receive or send calls w/full bars, cannot send receive pictures

I'm having an on going problem with my LG Prime, approximately one year old. In August I lost the ability to send and receive pictures/video from my home location. I can still make calls and send and receive regular text. The emblem on the phone went from an E to an antennae. I watched it change as I stood in my kitchen. I called AT&T who opened a case and said the Internet portion of my phone, which is used for pictures, was not connecting to the tower and they would look into it. They said they would call me back. The case was closed without a resolution. Incidentially, I can leave my home a few feet away, the antennae emblem on the phone goes away, I get the E emblem back and all is fine.
In addition, I drive 10 miles to work. Same county, different zip code. As of October 10, 2012, I can no longer make or receive calls, not even emergency calls, yet can text, receive and send pictures and video. A call to AT&T with all the usual resetting of phone, checking of cell towers, which is on the building where I work, produced nothing. I was told to get a new SIM card. After getting a new SIM card, I have the same problems. The phone rings, call shows on the display, minutes are used until the signal bars systematically drop off one at a time until the call is dead without ever hearing the person on the other end.
AT&T has said they will refund my money when the problem is resolved, but I have to keep paying the monthly $25 for them to keep telling me they don't know what the problem is and that they will get back to me.
If there is someone with knowledge or the ability to help, I would greatly appreciate it. Thank you.

Have you been able to try your simcard i another phone and see if the problems follows the simcard, if so what was the outcome, if not then could you give that a try when you get the chance, customer care eliminated the simcard issue but did not eliminate the phone as the issue, if putting the simcard in another phone and it works then a warranty claim should be done as long as you are not past the initial 365 days of purchase.

AT&T's return policy has changed effective 10/07/12. You have 14 days to return the device & cxl without a ETF. Policy info HERE.

Unfortunately, I don't have access to another phone, although I have asked around at work. As an additional note, while in a staff meeting, those not using AT&T GoPhones, were not having trouble with service. This may support the theory that pre pay phones get different service than post pay phones even when a tower is near. Although as you said, it doesn't rule out the phone itself. However, on Friday 9/28, I got intermittent service back at work. AT&T called me on the cell phone and although they were disconnected several times, I was able to make calls later that day. I don't know yet, if this has been fixed. But the E emblem is back at my home location instead of the antenna emblem. The emblem fluctuated during the day, but seemed steady throughout the weekend allowing me to send and receive files. Perhaps there has been a change on the network because it appeared I got service this weekend in some places (towns) that previously had none. Thank you for your time and warranty information. *I see in my previous message I wrote October 10 where it should say September 10.
