The activation fee is indeed a one time charge which is applied to your first bill. Everyone must pay this fee as its also outlined in the contract you signed. ;
The reason to pay this fee is because:
"The activation fee is a one-time fee that is charged to all new customers that activate a new line of service. These costs allow us to provide you with discounts on equipment, offer you great packages and features, and provide you with strong network coverage. This fee allows us to continue to provide you with excellent services now and in the future."

The activation/upgrade fee is per line/new phone. It is one time $36 fee but they give you $450 discount on those iPhones, each line would have a separate contract and a fee.
With that said, to clarify, are you saying that the fist line has one activation fee of $36 but other 2 lines have both activation and then an upgrade fees on the same bill? So the charges are for $36 five times? How and where did you start your plan and buy phones?

I bought iPhone and open account at apple store and after ten minute I must go to at&t counter for open another line because computer at apple store down but I can add only line so i must go back to apple store again with sim card for buy more iPhone when I got bill the line that I open and get phone at apple store it had only activation fee 36$ and another number had activation fee 36$ and one time upgrade 36$ . I would like to know the second line is fee double or not

Well if you moved from T-Mobile to AT&T, you would have been charged $36 for each of the lines your ported over.
Then when you purchased the iPhone 5, it counts as an upgrade, so you would have to pay a $36 fee for every iPhone you activated. So now you have two charges of $36 each. The first one for porting the line, the second for "upgrading" to the iPhone 5. If you bought two iPhone 5s, you would have this "double" charge twice (once for each upgrade).
