AT&T DSL Billing - Loyal customer lied to and now a collection notice. Can someone help??

AT&T DSL issue. We moved from Illinois to California in June 2011. Given the way AT&T is organized was told that my Illinois DSL account would be disconnected and a California account number would be created. We realized in Feb. 2012, that my credit card was continued to be billed from June 2011 to Feb. 2012 for DSL services in Illinois. At the same time I was also being billed in California for AT&T u-verse services. When I called AT&T in February 2012 and talked to them, the customer services representative agreed with me and tried to reverse charges. She assured me this will work. It failed. I've been talking to multiple representatives for over 6 months now. And each time I am assured that the charges would be reversed, but something happens in that big rat hole called AT&T corporate and now I have a collection notice from ATT for charges on the DSL line that I disconnected in June 2011.
I've since spoken to at least 3 or 4 customer service reps and they all agreed I've been double billed. But then I get a collection notice. What sort of way is this to treat a customer who has been loyal to AT&T?
No one can give me a straight answer. The shame of it is that we are still with AT&T. We've been with AT&T for over 10 years. I am seriously thinking of disconnecting not only my internet but also my wife's AT&T wireless account and will fork over the money to cable or Verizon. I've had it with AT&T.
Is there any one in AT&T who can help this loyal customer get the money owed?

Is there a way to reach the person in charge of customer service? Like the V.P of customer service?
