Why are so many text messages listed as To/From "phone"

It finally happened - I exceeded my 200/month quota of text messages. (iPhone 4S, grandfathered data plan)
When I look at the "Data Detail" for my messages, some of the numbers are familiar (my phone knows them), but many of the listings look like this (third and fourth columns different from other rows):

What is this? I have no easy way to check what I was doing at 10:44am this morning, but I don't *think* I sent a text message. Maybe I'm missing something, but I don't see it in the Messages app.
What am I being billed for?

If you have a data plan, that's fine.
If you don't have a data plan, and never use mobile internet, it probably comes from accidentaly hitting the browser button, and you will be billed at a rate of $2 per MB, or $2048 per GB. You should call 611 and put a data block on the line for free.

Be advised, though, that if you have a data block put on your line, then mms messaging won't work. MMS requires a working data connection to send/receive picture or video messages.
