VoiceMail Password not set up by us & Group Texts not done?

We have a iphone 4 for our daughter. We are set up for 300 MB and she has been going over. After the first month, we let her know and what she need to do to make sure this does not keep happening. She has been very careful about using it at home and just using phone and text when she is out. We started having overages again so are going over the bill. There were large amounts like 27,919 KB headed Internet/Media.Net at times like 2:00 AM when she does not have the phone. Thanks to this list I see that can be something like data transfer that AT&T does. It looks like we are being charged for it though. I have followed steps people recommended to help with this so we will see what happens with it.
We had never hooked up our daughters voice mail no had she. Now we notice that she has 4 voice mail messages but we need to use the password to get them but we never set it up so we have no idea what that password is. Our daughter did not set this up either. Anyone know how this could happen or how we could find the password?
I just did a check of her data usage and receive text message. It says again she is over but another mystery is Group Messaging of 1,791. She does not know how to send a group message. Any ideas on how this could happen?
She primarily uses the phone for text, phone and games. She had about 30 phone numbers in her phone but there are only about 8 she uses regularly.

For starters, the data usage is hard to diagnose but understand that data can be used quite a bit including email, web or even apps such as facebook, and even games. As for the time, ATT and others agregate the time that data is recorded so that could be the spike you see and it may not be to actual time of use. Search the boards here for other threads about data use to see what others have found.
If you truly want to limit cellular data use, go to settings>Cellular>cellular data and turn it off. While this defeats the purpose of a smart phone, wifi will still work and when she goes into apps that use data, they should give her a message about the lack of the cellular network and that may provide a clue as to what is the big culprit.

I woud suggest though that 300mb is probably not suffcient for most users.........
